That looks to be a Marelli Turbine manufactured around 1910. They are ceiling hung fans that rotate 360 degrees while in operation, the lower guard was added at a later date, likely to prevent guests wherever the fan was hung from accidentally walking into or putting their hand into the spinning blades, or protect the fan itself from damage. Not very common to find, very cool.
