Description
The card is the property of the Insurance Commissioners (England) and is issued for the use of an Employer when a Contributor has failed to deliver their Contribution Card. The card has a crest at the top. The words: Employed Contributor on the left hand side. England is printed on the right hand side. An explanation about the use of the card and allowances for stamps. There are two 10d National Health stamps affixed to the stamp section of the card. There is no signature on the card, however C. K. Mills' name is printed on the top right side.
